You will receive a confirmation letter by mail and/or email. If you do not receive a confirmation, please contact Ashley at APorter@northmetro.org or by calling 678-290-7830 x134.

*Once the application has been downloaded, use Microsoft Word to open the document. Complete the form by tabbing through the document and typing in the fields or clicking on a selection box. When complete, email the completed form to APorter@northmetro.org. If you have trouble with the form, contact Ashley Porter or Patricia Hurtt at the North Metro office (678-290-7830).

The forms are available at the Information Center in both lobbies and can be accessed by the links shown above. If you would like to have an application emailed or mailed to you, please contact Ashley Porter at 678-290-7830 x134 or by email at APorter@northmetro.org. Complete the appropriate form and submit it to the church office no later than the application deadline for the team for which you are applying. If there are more applicants than can be on the team, the date and time that the Application/Reactivation is received at the office can be a determining factor in who can and cannot go.
